Digital planning and guided surgeries

We have software that allows us to superimpose the 3D X-ray of the bone and the 3D scan of the teeth and gums, allowing us to plan the optimal position of the implants in relation to the bone that will hold them and the mechanics of the prosthesis.

We will then design and print a guide splint with the holes corresponding to the digital position of the implants, which we will place in the edentulous area of the patient before the intervention, a process known as guided surgery.

Immediate loading technique

Immediate loading consists of screwing a provisional prosthesis to the implant after surgery. Thanks to preoperative 3D buccal scanning, our laboratory will be able to design and fabricate it before the intervention in order to place it right after the surgery.

This option improves the final volume of the gum and its esthetics in relation to the definitive prosthesis, since it will start to be shaped just after the intervention through contact with the provisional. When the implant is osseointegrated and we take the records to make the definitive prosthesis, the shape and volume of the gum will already be definitive, thus being able to adjust the prosthetic design to the characteristics of the same.

Gingival grafts in implant sites

Connective grafts: These are gum grafts indicated in visible areas where maximum esthetic care is required. Tooth extraction, gum disease or occlusion problems can lead to gum recession. To recover the lost gum volume, we obtain gum from the palate, an area that regenerates, and graft it to the affected area.

In cases that do not require a very complex bone regeneration, it is indicated to perform the graft on the same day of the implant surgery. If yes, it will be carried out in a second phase. Immediately after the intervention we will place the provisional prosthesis so that it begins to adapt to the shape of the tooth.

Free gingival grafts: They are indicated when there is little gingiva in non-esthetic areas. They are performed to avoid the risk of perimplantitis: inflammation caused by bone loss around the implant.

Bone regenerations in implant sites

If we diagnose lack of bone through the 3D X-ray, we will perform a bone graft to restore bone volume and thus be able to place the implant. In the indicated cases, we will cover the graft with a membrane to stabilize the regeneration. The bone we graft is of bovine origin and, in some cases, we mix it with the patient’s own bone that we extract from a surplus area, which facilitates the integration process.

In case of dental absence or if the tooth to be extracted is not infected and has sufficient bone support, we can perform the extraction, bone grafting and implant placement in the same intervention, thus reducing the number of interventions and the time until the definitive prosthesis is placed.

In cases where the tooth to be extracted presents infection or does not have sufficient bone support, it will be necessary to wait between 2 and 3 months from the extraction until the regeneration and placement of the implant. In the indicated cases, in order to minimize the consequences of post-extraction bone height and width loss (alveolar ridge resorption), we will perform bone regeneration on the same day of extraction, an intervention known as alveolar ridge preservation.

When we perform the regeneration and implant placement in the same intervention, the time until the definitive prosthesis is placed is extended from 2/3 months to 4/5 months. When the implant placement is to be performed in the upper sinus area and bone regeneration is required, the bone and membrane will have to be introduced into these cavities, an intervention known as sinus lift. In these cases the time until the definitive prosthesis can be placed will be extended up to 6 months. It will be important not to take any airplane until 15 days after surgery to avoid problems with air pressure.
